Eligibility for Divyang Bharan Poshan Anudaan

(i) The age of the applicant should be less than 18 years.
(ii) The applicant should be a BPL card holder or the monthly income should be maximum Rs. 4000/- per month.
(iii) Disability should be 40 percent or more than 40 percent.

Documents required for online application

(i) Income certificate of Rs. 4000/- per month issued online from all sources or photocopy of BPL card.
(ii) Copy of family register issued by Panchayat Minister for applicants from rural areas or photocopy of ration card for applicants from urban areas.
(iii) Valid certificate of more than 40 percent disability from a government medical officer.
(iv) Copy of the proposal selected in the open meeting of Gram Panchayat/Councillor.
(v) Certified photo of the applicant (from Pradhan/Panchayat Minister/Sabhasad/Councillor).
(vi) Photocopy of CBS bank account passbook.
(vii) Photocopy of Aadhar card.
